Volunteering at School this year?
Whether you plan on helping in the class, garden, school event, field trip, or taking part in one of the other programs at school, you will need to fill out a volunteer application.
Volunteer forms need to be filled out every school year and must be accompanied by a copy of your TB test (read within the last four years) or your TB Questionnaire completed within the last four years.
We now have two volunteer designations at Silver Gate to comply with district policy.

If you plan on volunteering in any capacity, other than a field trip driver, you will need to submit all the Category B Volunteer requirements to the front office.

If you plan on driving on field trips, you will need to complete all the
Category D Volunteer requirements.
Please stop by the office to pick up the complete Category D packet.
TB TEST or QUESTIONNAIRE
All Volunteers must have
a current TB test (valid for four years from the date read) on file.
Please turn in a copy of your TB test if needed or if yours is set to expire.
The Adult Tuberculosis (TB) Risk Assessment Questionnaire may be turned in in lieu of the TB test results if it is administered and signed by a licensed healthcare provider (physician, physician assistant, nurse, nurse practitioner).
PLEASE NOTE, THESE ITEMS MUST BE TURNED INTO THE SCHOOL OFFICE PRIOR TO ANY VOLUNTEERING. IT TAKES APPROXIMATELY TWO DAYS TO COMPLETE THE CATEGORY B
